Here, you will complete a FREE intake form (on behalf of yourself or the living individual(s) needing end-of-life care). All information shared is kept confidential, except for emergency purposes. I will reply to your submission via email within three business days.
ON PAYMENT: There is no set fee for end-of-life and beyond care. Suggested scale of payment is between $60-300 per session. Payment may be submitted anywhere between being onboarded as a client and two weeks from the date of each session. A direct payment link and other payment platform options will be shared with you/the paying party.
