End-of-Life (and Beyond) Care

This page is for requesting end-of-life and beyond care in-person, on Zoom, over the phone, or over email.

Here, you will complete an intake form (on behalf of yourself, if you yourself are in need of end-of-life care or would like me to check on a deceased person OR on behalf of the living individual(s) needing end-of-life care) that will help me to see if I am a good match for you/the recipients. All information shared is kept confidential, except for emergency purposes. I will reply to your submission via email within three business days.

It is FREE to complete the form. There is no set fee for end-of-life and beyond care. Suggested scale of payment is between $60-300 per session. Payment may be submitted anywhere between being onboarded as a client and two weeks from the date of each session. A direct payment link and other payment platform options will be shared with you.
